Financial Assistance

If you are in need of financial assistance, please complete both of the following forms: 

  1) Federal Income Verification Form - English or Spanish

  2) Financial Assistance Application

These forms collectively help us determine your eligibility for assistance; both forms must be completed.  All information will be kept strictly confidential.  A new application must be submitted each school year.  For any questions or help completing the forms, please contact Janet Bealer, School Social Worker, at janet.bealer@lincolncharter.org.
